Is there a way to add a different OrderItemType without having CommerceBuilder source code? If not, can you suggest a way to handle something like that- be able to save an order item as a new type?

There is no other way so far as I know. Enum types cannot be extended or inherited. Even if you could extend it, there's a lot of internal logic specifically hardcoded to specific order item types. Like recalculating a basket deletes all shipping and tax items. That sort of thing.
